1. Open valid bin in extreme.

2. Under EEPROM drop down menu select store all.

3. Open blacklisted bin.

4. Under EEPROM drop down menu select patch all.

5. Save as bin file.

You may be trying too hard, too many different things. Try this, it is the accepted correct way to accomplish what you're trying to do.
1) Unloop your card
2) If you're going to patch a valid CamID and ZKT tables to another bin, follow Wasteospace's directions EXACTLY. No variations.
3) Write your patched bin to your card.
4) Clean your card with a proper cleaning utility.
5) Write a good 3m, or activation script, whatever you're using, to your card. DTV hashes 3m and act scripts every day, so don't automatically assume that it's good.
6) Properly reboot your receiver
7) Watch TV
There's no reason why that shouldn't work...unless one of your files is corrupt, hashed or blacklisted....
