First step make sure your programmer and connections are ok.
If you have another card try programming it to make sure the
equipment is working properly.
If the equipment checks out then the card is pretty messed up.
Especially reading 40 as a provider id.
Can you still do a read?
If you can you may still be able to write it.
Do you have a backup of the orginal image from this
card?
If so you may be able to save the card. Try writing
the orginal image back to the card. If that works try
a one step clean again.
WARNING READ VERY CAREFULLY YOU CAN KILL YOUR CARD WITH THIS METHOD. IF YOU DON'T UNDERSTAND IT DON'T TRY IT!
If that dosen't work I have found only one other way
short of sending it to a dealer for pro cleaning/unlooping.
Write small parts of the orginal info a little at a time. Until
enough is back to do a full write. Things like write just the code space of the card. Then write just the data area of the card.
Then the ird info. Then the provider info. etc.
Write small chunks until it's back to normal. The best way to
do this is create ep10 patches of the relevant areas. This does
require a very good understanding of the card itself. For the most part where specfic data is located and how to create a ep10 that will write it to the correct locations. And a understanding of more advanced features of nagraedit.
If you are not sure of what I am talking about above:
DO NOT TRY IT! YOU WILL KILL YOUR CARD!

Unusual error in nagraedit
The reason it fails to clean correctly is that during the clean process, it has to write the cleaned image back to the card, and its the writing that's actually failing. Usually what happens is that it will fail just after "disabling card", and the next time you try to write, it fails just after "enabling card" (it alternates back and forth). If it fails just after "enabling card", then just above that line you will see a message "writing codespace", and "writing dataspace". As long as you see those two lines, then the image was written OK.

Had the same problem with one of my rom 10's just click write again and it will say rom 10 written succesfully. after that make sure to do a read again to make sure it is locked before you put it into your receiver. With mine I had to One step clean twice in a row then write to card twice in a row have had no problem since hope this helps
Thanks guys for all the help
