Burlfish, try using another bin. DTV has swapped that card out with whomever it's subscribed to, and cancelled the CamID. That's why you're getting the message. Get a new valid bin, put it on the card along with a good 3m or activation script and you should be fine....
I did notice you said that your other cards are working OK. Do they have this same bin on them?

I wondered about that after I read your post...and it does seem as if it's related to that one card. Try this...if you have another bin, try it and see - on the one suspect card. Clean it thoroughly, carefully reapply everything and try it again - see if you can isolate exactly what's causing the problem. The IRD only knows what it sees - and it's seeing a CamID problem. Could be the bin or "trash" on the card...but if writing a known valid bin and a good script gives you the same message, then I'd suspect the card itself.
