HBO, Showtime and Cinemax keeps dropping

I have a subbed bin that was out of the stream for almost 2 months. I wrote it to the card after cleaning it 4 times. I created an act using Activation Workshop 2.3 from the subbed bin. After a few hours HBO, Cinemax and Showtime comes up 721. After a week of rewriting the act to the card I reinstalled the original bin, wrote the 0700 update hex to the card and edited byte 3224 to 00 to force a USW update through the stream to get valid group keys. I changed 3224 back to 02 after the 0800 update and recreated an act from the updated bin and wrote it to the card. Still same result. After several hours HBO, Cinemax and Showtime go 721. No problems with any other channels. Anyone have any suggestions?

Is there a way to manually activate these tiers? Writing an act hex to the card wipes out the 0006 DSW. Now that the 0006 update is out of the stream I keep getting Please Stand By. I tried using the USW/DSW hex that's out but it's not working for me.
