The Armulator.
I've read a review from Rotten and he says it is solid. What seems neat about this is that you program with your USB cable.

The Armulator is an ST MicroElectronics ST19 smart card emulator. It
currently supports emulation of both the the ST19CF68 and ST19XL18 series
of smartcards from ST MicroElectronics. It is a flash memory device
that allows the manipulation of both user rom and Eeprom on device, from
testing current publically available code that exists for both of these
smart cards. The most popular known company that uses these cards and
public documentation exists for these devices is NagraVision's series
of Pay-TV access cards.
Currently NagraVision uses the ST19CF68 in North America as one of
their newer series of access cards called the rom101,
