download "the mighty loader" and "mighty loader flashers" [get the xp flashers if you have win xp]

you must have a dtv unlooper and flash it with "MIGHTY S 5211" flash in order for the unlooper to read your dish card.

read the read me and use the "magic numbers" of
000391
&
000121.

uncheck boxes
"apply when writing" & "reverse ird number"
click on "get" and wait until your card info appears.
click on "UNLOCK"
when it unlocks the card, read it in MIGHTY LOADER" 1ST.
and save the bin.

after your card is unlocked, read it in nagra edit 3.0 and save the bin again as
Rom 10 - S 00 xxxx xxxx first read.bin

now you can do a full clean of the card and proceed to fixing your rom 10.
