Lock/Fuse Error ?

I seem to be locked out of my atmega when I try to Virginize my card in pheonix loader it says writing default fuses failed, lofuse verify failed. I tried to factory restore my card it failed as well it said fuse setting may need to be changed? not sure where to go from here. has anyone else had this problem?

try using jeepers
alternate chip erase and erase flash/eeprom
