Switch diagram needed for connecting FTA to 4DTV Receiver 922

Hi,

Does anyone have a link to a diagram that would show me how to connect my Captive Works CW800S FTA to my 4DTV 922 receiver. I have the disq switch and two splitters with dc power pass.
