This worked for me...go into ExtremeHu and edit your bin. Clear your password by typing 0000 and change the ratings limit to 00. You can look first by reading the bin on your card and checking PW and ratings. My password showed 6085, but it didn't unlock my channells. I changed these and everything worked fine. I couldn't get the PPVs.

00 is supposed to work; I personally use 0B and get all of the channels that I want. Hope this works for you!

Go into the edit mode (magnifying glass). Change your password then tab to the ratings and change it. Then go to File>Save as>bin and give it a new name.bin. Go back and write this bin to your card. I edited my zip code. local bytes and ratings. I'm using my subbed bin on Mili's template
